Onsite sewage permits are issued by county health departments under the Alabama Department of Public Health; we could not retrieve a published fee schedule for any county. What Decides Which Column You’re In
Rule 420-3-1-.25 lists the conditions. In residential terms, the ones that matter most:
Hard bedrock less than 12 inches from the natural ground surface.
Percolation slower than 240 minutes per inch — the heavy clay case, common across the Black Belt.
Percolation faster than 5 minutes per inch — the very sandy case, where effluent moves through too quickly for treatment.
Seasonal saturation within 6 inches of the natural ground surface.
Mine spoil at natural grade to 48 inches within the disposal field area.

Costs That Sit Outside the Installation Quote
The soil and site evaluation, performed by a registered professional such as an engineer or soil scientist, who conducts percolation tests or soil profile studies. This comes first and is generally billed separately.
Engineering, where required. ADPH permits only registered engineers to design systems where percolation exceeds 60 minutes or where there’s a shallow limiting zone such as groundwater, clay or bedrock. That design work is its own professional cost.
The permit fee to your county health department.
Access and restoration. Equipment has to reach the disposal field area, and the yard needs putting back afterward.
Replacement Is Evaluated Against Today’s Rules
It isn’t a loophole or a technicality — the standards changed because the older ones weren’t adequately protecting groundwater. But it does mean the honest answer to “what will this cost” is “it depends what the evaluation says,” and the evaluation is worth getting early.
Repairs Need a Permit Too
Under 420-3-1-.06, no person may begin the installation or repair of a system or component thereof until the responsible person has been issued a valid Permit to Install (Repair) by the local health department.
That’s a broader reach than several other states. If you’re doing anything beyond routine pumping, a call to your county health department settles whether a permit is needed — and that call is considerably cheaper than an unpermitted repair surfacing during a future sale.
Who Can Do the Work
Installation must be performed by an installer licensed by the Alabama Onsite Wastewater Board. Homeowners may not install their own systems — doing it yourself would require becoming a licensed installer, which means classes with a state-registered sponsor, a test, forms, fees and renewals.
Get more than one quote. The licensing requirement narrows the field, but the spread on identical work is still wider than most people expect.
